Item Name: Support Coordination Level 3: Specialist Support Coordination

Specialist support coordination may be provided to participants of the NDIS who require a higher level of support coordination for more complex and challenging needs.

If you have this funding, at Your Voice a specialist support coordinator with appropriate qualifications and experience will provide you or your family member with assistance to navigate and coordinate services during times of significant life transitions.

This may include referral to a range of services across the broader health, welfare and disability system, engagement in multidisciplinary team meetings and development of care and service plans.

Specialist support coordination is generally delivered through an intensive and time limited period, after which a support coordinator may take over the ongoing management of the support plan.

All aspects of support coordination Level 2 are also included in the provision of Level 3 specialist support coordination. It’s important to note that individual circumstances vary and no two participants will need exactly the same support plan.
